4、最高学历、学位证书复印件及翻译件/ A Photocopy of Diploma of the Most Advanced Studies and Chinese translation

5、成绩单的复印件及翻译件/ A Photocopy of Transcripts of the Most Advanced Studies and Chinese translation

 6、《外国人体格检查记录表》/Foreigner Physical Examination Form;

7、本人近期免冠照片(电子档) / Photo(electronic file)
